As pro-active measures, please cascade down this classic incident as lesson learnt to all personnel within your organization.

Frequent reminders are essential to ensure that they are fully understand the risk of radioactive exposure and its consequences.

Pls emphasised that ” if in doubt, and you are unauthorised person to remove, repair, alter and modify to any part of equipment or material at work place, pls seek advise of the competent and/or authorised person.
