Tripod Jacket Collapse Offshore During Piling Operations
The incident that i presented here was not suppose to be released to the public. But again, we are unable to control the World Wide Web. Just before we proceed, i would like to make my self clear that the video was hot linked from a PUBLIC DOMAIN website and uploaded by someone that i have no contact with. Lets view the video and i’ll try to reply on most of the comments made about it.
Upon receiving the news about the incident, the Operation Department forming a special team (Most of them are those who are directly involved in the projects from start) to brain storm on how to recover it back. The team successfully execute the job safely. In order to clear the air here is my reply on the public comments about the footage. (taken from the public domain as of this post was written)

Stupid fool… i wonder who gave this people handling PTW for this work. They should get the experience ones not the new guys. It’s not self accident scene situation but a human mistakes.

actually they are performing an istallation on what is called a tripod jacket or platform. the long piping sticking up in the air are pilings. sometimes when the supervision is gung ho they do stupid shit.they should have driven the first piling down before setting the others. by not doing so the platform became topheavy and you see the results. ive been on two of those when they topple over , pretty hilarious stuff lmao
(Your first line of comment was right. But next comment on piling sequence is not accurate. Piling sequence was based on Engineering On Bottom Stability & taken into consideration other parameters i.e. Clients soil report, design input, allowable wave height etc…)

That is an oil rig collapsing. It was a prop up rig, one that stands up when it is at its destination. one legs broke so it fell.
(This is the funniest comment of all.)
I wish i can write more about it. About the investigation result, about a special paper written about it – which the client request IEM not to be publish it. Below is my picture taken during recovery of the tripod jacket.

Hi..Mr. Sulaiman..ya I saw this video in Youtube a year back..this suppose to be confidential, but not sure the person who released this to public…yes agree with you, as ppl who dont know about offshore installation will comment (some are pretty funny)…yes things like that happen offshore and sometimes alot of factors involved, this is real engineering not textbook science (everything seems perfect)…what to do…as installation contractor ppl will always try to blame..

Iyup, i even sent an email to the owner to take down the footage but no luck. From there i got a lot of email asking about the footage since i was directly involved during the special recovery operations.
It is pretty annoying reading different kind of comments from public and again as the contractor we are obliged not to give more details about it . I hope this post will clear the air.
fnd : it is nice to read comments from people who is really understand the nature of Offshore Construction Operations.

Iyup, the jacket is softmoored (2 nos) to the stern of the barge. (I cannot elaborate more. Because this was one of the major issue during (till now) the investigations.)
BTW, Due to this post also, i was called by the top management & they want explanation why i post the video here. Its a web technical issue since the video is not actually hosted here. The video was uploaded by somebody else (Which i dont have contact with) & in a public domain web (YouTube) – as i state clearly in the above article. But again, the motivation behind this post is nothing but to give some brief explanation on what is actually happen.(without giving too much detail that might do harm to the company & even the investigation). – notice some funny comments from user which is really annoying.

What is the water depth that you are working in? I would recommend next time pre installing a single pile at the location prior to the platforms arrival. Have INVERTED BELL GUIDES installed in the center of the platform. Stab the platform over the pre installed pile. After the pin piles are driven, the center pile may be used as a well conductor.

To set the a jacket onto pre-drilled well head is easy if the seabed is in even condition. However, I’ve experienced difficulty to level the jacket as the seabed was uneven and we still have to set down the jacket perfectly onto the pre-installed well head. In summary, you have no option to relocate the jacket if the seabed condition not at your side.
